Decision Support Tool for Advanced Long Term Care Planning

Using an evidence-based approach to supporting well-informed health care decisions, this project is developing, testing, and evaluating an interactive decision support tool on the National Long Term Care Clearinghouse website to help families learn about and choose the best long term care services and financial plans to meet their needs and preferences.


Decision Support Tools for Caregivers and Patients with Alzheimer’s Disease

Decision support interventions have been shown to help caregivers of patients with advanced dementia imrpove their understanding of the medical information, form realistic expectations, clarify personal values about the risks/benefits of treatments, and communicate with their health care team.  This project seeks to explore the decision support needs and preferences of patients and caregivers across the spectrum of early/mid/late-stage Alzheimer’s Disease and to identify effective strategies for the design and implementation of decision support programs earlier in the disease process in order to foster patient-caregiver communication and advanced care planning, and to reduce caregiver burden.
